  • Use the form below to ask our Resource & Referral Team for help. This form is the first step to getting support from Disability Rights Tennessee, including reporting abuse or neglect. Before you fill it out, here are a few things to know:

    • If you are reporting abuse or neglect, you can also contact Adult Protective Services (1-888-277-8366) or Child Protective Services (877-237-0004) directly.
    • This form is for general help requests — things like legal advocacy, TennCare Long Term Support Programs, the Katie Beckett Program, CHOICES, and ECF CHOICES.
    • We can’t help with every situation. If we’re not the right fit, you can visit our Resource Hub, call 211, or reach out to another legal aid group.
